Templating a Complex Modular Stem for a THR on a Dysplastic Hip and Planning a Subtractive Osteotomy

OrthoView is able to present the full range of options and compatibilities available with complex modular prostheses on-screen so that the surgeon does not need to memorize them.  The surgeon is also able to identify that the correct amount of offset is achievable with the chosen prosthesis.

One-click scaling with QuickScale in OrthoView allows both the appropriate template size to be chosen and the pre-operative leg length difference (in this case 20 mm) to be identified. A suitable correction method can then be chosen and tested on-screen.

A subtractive osteotomy was planned in OrthoView to correct the leg length difference. The position and amount of the resection is identified in OrthoView as can be seen in the templated image.

The patient was a female in her late 40s with a long history of DDH. Previous surgery on the right hip is evident in the pre-operative x-ray below. However on the left hip, the need for a total hip replacement was identified due to intractable pain.

A subtractive osteotomy was required to shorten the left leg by 20mm - correcting the pre-operative difference and compensating for any lengthening that might result from the THR.

For this patient, the DePuy S-ROM modular stem was chosen for its wider range of sizing options, including independent sizing of sleeve and stem. In addition, the surgeon is able to confirm the correct amount of offset could be achieved within the S-ROM system.
